This battalion was first formed on May 30, 1864, with three (3) companies:

Company A—Guilford County—Capt. John W. Pitts; 1st Lt. Jahew N. Crouch; 2nd Lt. George M. Glass; 3rd Lt. Thomas A. Glass (deserted), replaced by 3rd Lt. F. T. Tilly.

Company B—Alamance County and Forsyth County—Capt. A. L. Lancaster; 1st Lt. A. M. Craig; 2nd Lt. William S. May (died); 3rd Lt. Charles B. Pfohl, promoted to 2nd Lt.

Company C—Stokes County and Person County—Capt. Robert F. Dalton; 1st Lt. Glaucus Mason; 2nd Lt. George W. Yancey; 3rd Lt. John H. Shackelford.

On June 15, 1864, a fourth company was added to this battalion:

Company D—Rockingham County—Capt. Alfred B. Ellington; 1st Lt. John P. Ellington (drowned), replaced by Frank M. Hamlin (was 2nd Lt); 2nd Lt. Frank M. Hamlin, replaced by William J. Fewell (was 3rd Lt); 3rd Lt. William J. Fewell, replaced by Benjamin H. Boisseau.

John M. Reece, of Guilford County, was elected Major on May 30, 1864. He was captured at the battle of 1st Fort Fisher on December 25, 1864.
John Shaw Pescud was appointed Adjutant around June 10, 1864. He too was captured at the battle of Fort Fisher on December 25, 1864.

On January 3, 1865, this battalion was consolidated with the 7th NC Battalion - Junior Reserves and the 8th NC Battalion - Junior Reserves to create the 3rd NC Regiment Junior Reserves.
